Cancer cells have a protein called <a href=\"\/treatment\/immunotherapy\/pd1-and-pdl1-inhibitors\/\">PD-L1<\/a>, while T cells have another protein called PD-1.<\/p>\n<p>T cells are naturally programmed to seek out and destroy cancer cells. In the groundbreaking DREAM clinical trial, the medication stopped the disease from progressing for 6 months in nearly 60% of patients when used with chemotherapy. These trials are studying different ways to use durvalumab that could improve patients\u2019 <a href=\"\/mesothelioma\/prognosis\/\">mesothelioma prognosis<\/a> (overall health outlook).<\/p>\n<p><strong>Durvalumab is being tested:<\/strong><\/p>\n<ul>\n<li>As a first-line treatment (initial one used)<\/li>\n<li>As a second-line or third-line (follow-up) treatment<\/li>\n<li>By itself<\/li>\n<li>With other immunotherapies like <a href=\"\/treatment\/immunotherapy\/tremelimumab\/\">tremelimumab (Imjudo\u00ae)<\/a><\/li>\n<li>With other mesothelioma treatments like surgery or chemotherapy<\/li>\n<\/ul>\n<p>Most clinical trials for durvalumab are only available to patients diagnosed with malignant <a href=\"\/mesothelioma\/pleural\/\">pleural mesothelioma<\/a>. Learn about what these trials found when testing durvalumab alone and in combination with other treatments.<\/p>\n<h3>DREAM Trial<\/h3>\n<ul>\n<li><strong>Treatments tested:<\/strong> Durvalumab and chemotherapy<\/li>\n<li><strong>Type of mesothelioma:<\/strong> Pleural mesothelioma<\/li>\n<li><strong>Results:<\/strong> Durvalumab used with chemotherapy improved survival<\/li>\n<\/ul>\n<p>The DREAM trial studied how durvalumab could help pleural mesothelioma patients live longer when administered alongside <a href=\"\/treatment\/chemotherapy\/\">chemotherapy<\/a>. It was conducted between 2016 and 2017.<\/p>\n<p>This phase II study revealed that patients had an overall survival of <strong>18.4 months on average<\/strong> when durvalumab and chemotherapy were combined, with a 2-year <a href=\"\/mesothelioma\/prognosis\/survival-rate\/\">survival rate<\/a> of 37%. This is the percentage of patients still alive after 2 years.<\/p>\n<p>The trial also showed that durvalumab greatly slowed the spread of cancer, as 31 of the 54 patients in the study had a progression-free survival of 6 months.<\/p>\n<h3>PrE0505 Trial<\/h3>\n<ul>\n<li><strong>Treatments tested:<\/strong> Durvalumab and chemotherapy<\/li>\n<li><strong>Type of mesothelioma:<\/strong> Pleural mesothelioma<\/li>\n<li><strong>Results:<\/strong> Durvalumab added to chemotherapy improved survival<\/li>\n<\/ul>\n<p>The PrE0505 trial investigated if combining durvalumab with pemetrexed and cisplatin or carboplatin (commonly used chemotherapy drugs) could help patients with advanced pleural mesothelioma live longer.<\/p>\n<p>Some patients in the study received both durvalumab with chemotherapy, while others only received chemotherapy. Patients treated with durvalumab had longer life expectancies and higher survival rates.<\/p>\n<p>The results of the study were published in 2021. The average <a href=\"\/mesothelioma\/prognosis\/life-expectancy\/\">life expectancy<\/a> for patients who received durvalumab was <strong>20.4 months<\/strong>, while patients who received chemotherapy alone lived for 12.1 months.<\/p>\n<p>The 2-year survival of patients treated with durvalumab was 44.2%, and the 6-month progression-free survival rate was 67.3%.<\/p>\n<h3>DREAM3R Trial<\/h3>\n<ul>\n<li><strong>Treatments tested:<\/strong> Durvalumab, chemotherapy, and the immunotherapy drugs Opdivo and Yervoy<\/li>\n<li><strong>Type of mesothelioma:<\/strong> Pleural<\/li>\n<li><strong>Results:<\/strong> To be announced (study in process)<\/li>\n<\/ul>\n<p>The phase III DREAM3R trial began in 2021 following the encouraging results from phase II.<\/p>\n<p><strong>In this study, pleural mesothelioma patients are being treated with one of the following:<\/strong><\/p>\n<ol>\n<li>Durvalumab with cisplatin\/carboplatin or pemetrexed<\/li>\n<li>Chemotherapy with pemetrexed and cisplatin\/carboplatin (no immunotherapy)<\/li>\n<li><a href=\"\/treatment\/immunotherapy\/opdivo-yervoy\/\">Opdivo (nivolumab) and Yervoy (ipilimumab)<\/a>, an immunotherapy combination recently approved by the U.S. Food and Drug Administration (FDA)<\/li>\n<\/ol>\n<blockquote><p>\u201cHopefully, with this study, we can get a definitive answer to the question of which is better: chemoimmunotherapy or ipilimumab and nivolumab.\u201d<br \/>\n<strong>\u2014 Dr. Christina S. Baik, mesothelioma specialist at Fred Hutchinson Cancer Center<\/strong><\/p><\/blockquote>\n<p>The study is expected to conclude in 2025, according to the National Cancer Institute (NCI). A specific median life expectancy wasn\u2019t listed for patients who received the combination immunotherapy. This was followed by 9 doses of durvalumab, with one dose administered every 4 weeks.<\/p>\n<p>The median overall survival of patients was <strong>16.5 months,<\/strong> with an average progression-free survival of <strong>5.7 months<\/strong>.
