They have shown promise in treating mesothelioma, a rare and aggressive cancer caused by asbestos exposure.<\/p>\n<p>Mesothelioma cancer cells can hide from T cells (which your immune system makes to fight cancer) by using substances called checkpoint proteins. To do this, it uses checkpoint proteins.<\/p>\n<p>These proteins tell your body to make T cells if it detects an illness like cancer, and also help the T cells know which cells are healthy and which ones are harmful.<\/p>\n<p>However, mesothelioma cells can produce proteins of their own that allow them to avoid being destroyed by T cells, according to the American Cancer Society (ACS).<\/p>\n<blockquote><p>\u201cOur immune systems are dormant until there is a need to fight foreign bodies, like an infection. Because cancer hides from the immune system, it doesn\u2019t know it needs to go to work.\u201d<br \/>\n<strong>\u2013 Dr. Stephen Lemon, medical oncologist (cancer doctor)<\/strong><\/p><\/blockquote>\n<p>Using different immunotherapy checkpoint inhibitors, doctors can ensure that your body\u2019s T cells can find and destroy mesothelioma.<\/p>\n<h2>Types of Immunotherapy Checkpoint Inhibitor Drugs<\/h2>\n<p>There are several types of checkpoint inhibitor medications that can treat mesothelioma. Each drug falls into a different category depending on which immune checkpoint protein it targets. Learn about each below.<\/p>\n<h3>CTLA-4 Inhibitors<\/h3>\n<p>T cells use a protein called CTLA-4 to find cancer cells. However, mesothelioma cells can bind proteins they make to the CTLA-4 receptors of T cells and avoid destruction. <a href=\"\/treatment\/immunotherapy\/ctla4\/\">CTLA-4 inhibitors<\/a> prevent this.<\/p>\n<p>Two anti-CTLA-4 medications for mesothelioma are Yervoy and Imjudo.<\/p>\n<h4>Ipilimumab (Yervoy\u00ae)<\/h4>\n<p>Yervoy is currently used alongside another immunotherapy called nivolumab (Opdivo\u00ae) to treat malignant <a href=\"\/mesothelioma\/pleural\/\">pleural mesothelioma<\/a>.<\/p>\n<p>The breakthrough Checkmate 743 trial found that <a href=\"\/treatment\/immunotherapy\/opdivo-yervoy\/\">Opdivo and Yervoy<\/a> helped patients live for <strong>over 18 months on average<\/strong>. These immunotherapies were given prior to thoracic (chest) surgery.<\/p>\n<p>Patients who received both Imjudo and Imfinzi lived longer than those who just received Imfinzi.<\/p>\n<h3>PD-1 Inhibitors<\/h3>\n<p>A protein called PD-1 found on T cells can bind to another protein called PD-L1 on cancer cells. When this happens, the T cells won\u2019t kill cancer. Anti-PD-1 immunotherapies block this and stimulate the immune system so the T cells can destroy more of the cancer.<\/p>\n<p>Keytruda and Opdivo are examples of <a href=\"\/treatment\/immunotherapy\/pd1-and-pdl1-inhibitors\/\">PD-1 inhibitors<\/a> that can treat mesothelioma.<\/p>\n<h4>Nivolumab (Opdivo\u00ae)<\/h4>\n<p>Opdivo is an immunotherapy drug approved by the U.S. Food and Drug Administration (FDA) as a first-line treatment for pleural mesothelioma. It&#8217;s administered in combination with Yervoy to help patients who aren&#8217;t eligible for <a href=\"\/treatment\/surgery\/\">mesothelioma surgery<\/a>.<\/p>\n<blockquote><p>\u201cOpdivo and Yervoy yields durable survival, and we can see about a quarter of our patients are still alive and doing well at 3 years.\u201d<br \/>\n<strong>\u2013 Dr. Hedy Lee Kindler, mesothelioma specialist<\/strong><\/p><\/blockquote>\n<p>Since Opdivo targets PD-1 and Yervoy blocks CTLA-4, the overall immune response rate is greater than if either drug were used alone, helping to improve patient life expectancy in the process.<\/p>\n<h4>Pembrolizumab (Keytruda\u00ae)<\/h4>\n<p><a href=\"\/treatment\/immunotherapy\/keytruda\/\">Keytruda<\/a> was approved by the FDA as a first-line treatment for advanced cases of pleural mesothelioma in September 2024.<\/p>\n<p>According to a published in <em>The Lancet<\/em>, pleural mesothelioma patients who received Keytruda and chemotherapy lived for <strong>17.3 months<\/strong> \u2014 over a month longer than those treated with just chemotherapy.<\/p>\n<p><a href=\"\/mesothelioma\/peritoneal\/\">Peritoneal mesothelioma<\/a> patients treated with Keytruda had a median overall survival of <strong>20.9 months<\/strong> in a March 2023 study published in <em>Oncology<\/em>.<\/p>\n<h3>PD-L1 Inhibitors<\/h3>\n<p>PD-L1 inhibitors work very similarly to PD-1 inhibitors. Instead of blocking the PD-1 protein from T cells, though, PD-L1 produced by the mesothelioma cells is targeted. PD-L1 is a tumor-specific antigen, meaning it\u2019s a substance found on mesothelioma cells but not on healthy ones.<\/p>\n<p>Tecentriq, Bavencio, and Imfinzi are the three most notable anti-PD-L1 drugs being studied to treat mesothelioma.<\/p>\n<h4>Atezolizumab (Tecentriq\u00ae)<\/h4>\n<p>A 2021 study published in <em>Cancer Discovery<\/em> found that 85% of peritoneal mesothelioma patients were still alive a year after being treated with Tecentriq and another medication called <a href=\"\/treatment\/immunotherapy\/bevacizumab\/\">bevacizumab (Avastin\u00ae)<\/a>.<\/p>\n<p>The 1-year progression-free <a href=\"\/mesothelioma\/prognosis\/survival-rate\/\">survival rate<\/a> was 61%, meaning the cancer stopped spreading in more than 6 out of 10 patients.<\/p>\n<h4>Avelumab (Bavencio\u00ae)<\/h4>\n<p>Pleural mesothelioma patients treated with <a href=\"https:\/\/www.mesotheliomahope.com\/treatment\/immunotherapy\/avelumab\/\">Bavencio<\/a> and radiation therapy lived for <strong>over 1 year on average<\/strong>, according to a 2023 study from <em>JTO Clinical and Research Reports.<\/em><\/p>\n<p>All of the patients in the study were previously treated with chemotherapy, but their cancer had started to come back.<\/p>\n<h4>Durvalumab (Imfinzi\u00ae)<\/h4>\n<p><a href=\"\/treatment\/immunotherapy\/durvalumab\/\">Imfinzi<\/a> is another type of immunotherapy checkpoint inhibitor made by the company AstraZeneca. The 2021 PrE0505 trial tested how Imfinzi could help pleural mesothelioma patients live longer.<\/p>\n<p>This study combined Imfinzi with chemotherapy, and patients had an average <a href=\"\/mesothelioma\/prognosis\/life-expectancy\/\">mesothelioma life expectancy<\/a> of <strong>20.4 months<\/strong>.<\/p>\n<p>Another 2021 study, the NIBIT-MESO-1 trial, showed that Imfinzi worked to treat both pleural and peritoneal mesothelioma patients. Numerous clinical trials have shown that using checkpoint inhibitors as part of a larger <a href=\"\/treatment\/\">mesothelioma treatment<\/a> plan can help improve overall survival.<\/p>\n<p>As of 2026, the FDA has approved <strong>Opdivo, Yervoy, and Keytruda<\/strong> as first-line immune checkpoint inhibitors for mesothelioma. However, you or a loved one may be able to access other types of immune checkpoint inhibitors through clinical trials.<\/p>\n<h2>Clinical Trials Testing Checkpoint Inhibitors and Mesothelioma<\/h2>\n<p>Many <a href=\"\/treatment\/clinical-trials\/\">mesothelioma clinical trials<\/a> focus on immune checkpoint inhibitors and other immunotherapies. These trials study how checkpoint inhibitors for mesothelioma could help patients live longer, better lives.<\/p>\n<p><strong>Notable ongoing clinical trials for mesothelioma checkpoint inhibitors include:<\/strong><\/p>\n<ul>\n<li><strong>Alliance A092001 trial:<\/strong> This phase II trial will determine if using Tecentriq, Avastin, and chemotherapy will help patients live longer than if they received just chemotherapy and Avastin. It is sponsored by the National Cancer Institute (NCI).<\/li>\n<li><strong>DREAM3R trial:<\/strong> This phase III trial is studying if Imfinzi and chemotherapy will improve the life expectancy of pleural mesothelioma patients compared to those treated with just chemotherapy or Opdivo and Yervoy.
